WHEN THE JOURNEY IS THE HARDEST PART

A more manageable way to be seen.

A home visit may be worth discussing for an older adult with limited mobility, someone managing several health concerns, a person coming home after hospitalization or rehabilitation, or a family struggling to arrange transportation.

Initial, one-time and recurring visits are available subject to clinical fit and scheduling. What can be done at home depends on your needs and the visit arrangement. Call Presence to confirm availability for your location.

LOCAL ARRANGEMENTS

Thoughtful care, arranged around real needs.

Our office is in Hurst; home-visit conversations also include nearby Mid-Cities and the Haslet/North Fort Worth home market. Availability depends on the person’s needs, geography, and scheduling—not every area or concern is suited to a home visit.

For residents of assisted living, independent living, or memory-care communities, care arrangements may be explored with the resident, family, and community team as appropriate. Presence does not treat a home or community visit as a substitute for emergency care.

COMMON QUESTIONS

What else would you like to know?

Who is a good fit for a nurse practitioner home visit?

Older adults with limited mobility, people managing several ongoing conditions, patients recovering after a hospital or rehabilitation stay, and families who find transportation difficult often benefit most. Suitability is confirmed individually.

Are one-time home visits available?

Yes. Initial, one-time and recurring visits are available, subject to clinical fit and scheduling. Initial visits are $349; call 817-961-1100 to confirm current pricing and availability for your location.

Can home visits be part of a membership?

Yes. Home visits can be one-time care or part of an ongoing membership arrangement, so chronic care management stays connected with a provider who knows you. Call Presence to discuss the options.
