Primary care that makes room for you.
Meet a provider who listens, remembers your story, and helps you care for your health over time. Thoughtful primary care for adults and older adults, led by Melissa House, APRN-CNP.
Become a patientYour primary-care home starts with being known.
A primary care provider helps you make sense of your health over time: preventive visits, new concerns, ongoing conditions, medications, and what to do next. At Presence, those conversations begin with your priorities and the life behind your chart.
At a first ongoing-care visit, bring your questions, current medicines and relevant records. Talk with Melissa about your goals, what has changed, and which concern deserves attention first; agree on a next step to revisit. Her adult-gerontology focus brings particular understanding to changing needs later in life, while welcoming adults at many stages.

Prevention & everyday health
Make space for preventive care, changes you have noticed, and the questions you want answered.
Ongoing conditions
Work through chronic concerns with a provider who can follow the bigger picture across visits.
Medications & next steps
Discuss medication questions and coordinate appropriate follow-up as needs change.
One relationship. More than one setting.
In-person appointments are available at our Hurst location on Fridays and Saturdays, with appropriate virtual visits between. For some people, particularly older adults, a home or senior-community visit may be a better fit. Discuss visit arrangements and availability with Presence; home care is not promised in every area.
Our Hurst office is close to North Richland Hills, Bedford, Richland Hills, and other Mid-Cities neighbors. Haslet and North Fort Worth/Alliance are the practice’s home market, with Keller connecting both communities. Presence welcomes conversations from nearby Tarrant, Wise, and Denton-border communities; care setting and availability are confirmed directly with the practice.
For enrolled families, an unlimited-care membership adds 24/7/365 virtual-care access through the membership care network—another way to get care between primary-care visits.
A plan that follows the person, not just the appointment.
Begin with what matters to you; then talk through prevention, ongoing concerns, and a next step that can be revisited as your needs change.
