In-person Medicare primary-care appointments are available at our Hurst location on Fridays and Saturdays—close to North Richland Hills, Bedford, Richland Hills, and Keller. Appropriate virtual visits may also help maintain continuity. Home and senior-community visits are considered according to clinical needs, location, and availability.

After a hospital or rehabilitation stay, bring discharge instructions and a current medicine list if available. A primary-care follow-up can review what changed, questions about medicines, specialists or tests still due, how things are going at home, and a sensible next step. Discuss visit options and Medicare plan participation directly with Presence; the separate family virtual membership and relationship/DPC arrangements are different care options.
