FOR RESIDENTS & FAMILIES

Familiarity makes the details easier to hold.

Primary care in a senior community should still begin with the resident’s goals, preferences, and story. Presence welcomes conversations about care for people in assisted living, independent living, and memory-care settings when appropriate arrangements are possible.

Melissa House, APRN-CNP, is an Adult-Gerontology Primary Care Nurse Practitioner with a focus on older adults and ongoing conditions. With a resident’s permission, family members and caregivers can contribute to discussions about changing needs, medications, and follow-up.

FOR COMMUNITY TEAMS

A thoughtful point of connection.

01

Continuity

A primary-care relationship can help keep the resident’s history, goals, and ongoing concerns in view over time.

02

Coordination

Discuss medication questions, chronic conditions, specialist input, and follow-up after hospital or rehabilitation transitions.

03

Communication

When authorized, align care conversations with residents, families, and wellness or nursing teams around appropriate next steps.

DISCUSS AN ARRANGEMENT

Local partnerships begin with a conversation.

Executive directors, administrators, and wellness teams can speak with Presence about the needs of their residents and whether on-site primary-care visits are feasible. Any visit arrangement depends on clinical fit, location, capacity, and availability; no facility relationship is presumed.

Hurst and nearby Mid-Cities communities are closest to our Hurst location. Haslet and North Fort Worth/Alliance are its home market, with Keller between them. Care outside those areas is considered individually rather than promised across every location.

YOUR NEXT STEP

Let’s talk about the people in your care.

Residents, families, and community leaders can call Presence to discuss appropriate care arrangements.
