YOUR PROVIDER AT PRESENCE

Meet Melissa House.

APRN-CNP · Adult & Gerontology Primary Care

Care begins with being present.

Melissa House, a Board-Certified Adult-Gerontology Primary Care Nurse Practitioner, is the clinical leader of Presence Medical Care and provides personalized, relationship-centered primary care alongside Dr. Derek H. Lang, DO, Family Medicine. She cares for adults and older adults with attention to the person behind the chart—not only the reason for today’s visit.

HER APPROACH

To know your care, she wants to know you.

Listening is part of clinical care. Melissa makes space for your questions, your priorities, and the health experiences that brought you here. That fuller picture helps guide care as your needs change.

Her adult-gerontology focus is especially relevant to people managing ongoing conditions and families navigating care for an aging parent. Reviewing medications and following up as needs change can be part of that continuing primary-care conversation. Adults of different ages can find a primary-care relationship here; Presence is not only for seniors.

Presence connects the Hurst area with its Haslet and North Fort Worth home market. Melissa sees patients in person at our Hurst location on Fridays and Saturdays, and can talk with you about the right virtual or home-visit option; arrangements depend on availability.

CARE WITH MELISSA

A steady presence through changing needs.

Primary care, support for older adults, and ongoing conversations about chronic conditions all start with knowing the person.

Office and virtual visits are part of the care conversation. Home and senior-community visits depend on individual needs and availability.
