FOR YOU AND THE PEOPLE YOU CHOOSE

Aging is personal. Your care should be, too.

Good older-adult primary care begins with the person: what you want to keep doing, what feels different, and which concerns deserve attention. Melissa House, APRN-CNP, is an Adult-Gerontology Primary Care Nurse Practitioner; her adult and older-adult focus brings context to those conversations without treating age itself as a diagnosis.

If you are helping a parent, partner, or loved one, a provider who can hold the whole picture in view matters. With the patient’s permission, caregivers can share observations and take part in care conversations while the patient’s preferences lead.

MORE THAN A SINGLE CONCERN

Room to see what is changing.

01

Health & function

02

Medicines & conditions

Review medication lists and ongoing conditions together, including questions about how different treatments fit into daily life.

03

Transitions & support

Make space for follow-up after hospital or rehabilitation stays and coordinate with other clinicians and chosen caregivers where appropriate.

LOCAL CARE, CONTINUING RELATIONSHIPS

A primary-care home near Hurst.

Older adults and families from Hurst, North Richland Hills, Bedford, Richland Hills, and Keller can see Melissa in person at our Hurst location on Fridays and Saturdays. Appropriate virtual, home, or senior-community visits may be discussed based on needs, geography, and availability.

For adults with several conditions, continuity means fewer disconnected conversations: the same primary-care relationship can consider changes, medications, goals and recommendations together. After hospital or rehab care, discuss changed instructions, medicines, function at home and the next follow-up.
