For Employers · Plan Alternatives
An alternative to traditional group health insurance
If the group quote came back at a number your business cannot carry, the real choice is usually not plan A versus plan B. It is between a rich plan you cannot fund and nothing at all. There is a third option — as long as you understand exactly what it is.
Not insurance · Fixed per-member monthly cost · No underwriting, no renewal surprise
The short answer
- Who this is for
- Owners of small and midsize businesses who have priced group coverage and cannot make the numbers work.
- What a membership does
- Makes everyday care — 24/7 virtual visits, primary care, labs, imaging and medications — cheap, fast and predictable for your whole team.
- What it does not do
- It is not insurance and does not cover hospitalization, surgery, emergencies or specialists. It is not minimum essential coverage.
- What it costs
- A flat per-member monthly rate that does not change at renewal based on your team's claims.
- Next step
- Get a per-member quote and compare it against your group proposal.

What are you trying to solve?
- Get employer pricing
You cannot afford group coverage but want to offer something real
Per-employee memberships give 24/7 virtual access and member lab pricing at a fraction of premium cost.
- Hourly and part-time teams
You have a mostly hourly or part-time team
Coverage that works for teams who churn, with no enrolment windows.
- Get employer pricing
You want to add to an existing plan, not replace it
Memberships sit alongside a high-deductible plan and absorb the everyday care.
Memberships are not insurance and are not a substitute for major medical coverage.
Why the group quote keeps coming back too high
Small groups get priced on a small risk pool. One employee with a serious condition, one bad claims year, or simply a demographic mix the carrier does not love, and the renewal moves in a direction the business cannot absorb. Owners respond by raising the deductible until the plan technically exists but nobody can afford to use it — a $6,000 deductible plan for a $22-an-hour employee is a premium deduction, not healthcare.
That is the specific failure a membership addresses. It does nothing about a hospitalization. It does a great deal about the ten to fifteen ordinary medical events a household actually has in a year.
Fixed cost, no claims experience
The monthly rate is per member. It does not re-rate because someone on your team got sick.
Used, not just held
Unlimited 24/7 virtual visits with no copay for included care means employees use it for the small things instead of deferring.
Transparent downstream pricing
Labs, imaging and medications are quoted at member rates before anything is ordered, so employees are not opening surprise bills.
Where a membership fits next to real coverage
The most defensible structures we see are these, in order of how common they are:
Membership instead of nothing
The business genuinely cannot fund group coverage. A membership gives every employee real access to a provider rather than an empty benefits line. This is the majority case.
Membership under a high-deductible plan
You keep a lean major-medical plan for catastrophic risk and add a membership so routine care happens below the deductible without cost anxiety.
Membership for the ineligible group
Group plan for benefits-eligible full-time staff, membership for part-time, seasonal and 1099 workers.
Membership as the recruiting layer
Where competitors offer nothing, the ability to say every employee and their family has 24/7 access to a provider is a hiring advantage at a knowable cost.
The honest comparison
We will not tell you a membership replaces insurance, because it does not, and any vendor who tells you otherwise is one you should walk away from.
A membership handles
- Illness, infections and same-day medical questions, 24/7
- Ongoing conditions like hypertension, diabetes and thyroid follow-up
- Prescriptions and refills, with member pricing on medications
- Labs and imaging at member rates through participating partners
- In-person primary care with the same provider, in the Fort Worth area
Only insurance handles
- Hospitalization and surgery
- Emergency room and ambulance costs
- Specialist care and advanced treatment
- Catastrophic financial risk
- ACA minimum essential coverage requirements
If your business is an applicable large employer, treat this as an additional benefit and take compliance advice from your broker or counsel.

Questions people ask
Is this a health share or an indemnity plan?
Neither. It is a direct membership with a healthcare practice: you pay a monthly fee and receive defined services. There is no risk pool, no reimbursement process and no claims.
Can we pay for it with pre-tax dollars?
Tax treatment depends on your structure and how the benefit is funded. See our HSA/FSA information page and confirm with your accountant before making assumptions.
What happens at renewal?
There is no underwritten renewal. Pricing is per member and does not re-rate on your team's claims history.
Can we run this alongside a group plan?
Yes, and many employers do — most often as the layer covering employees who are not eligible for the group plan.
About this information
Written by the Presence care team for patients considering care with us. Presence care is delivered by licensed providers, including a board-certified Adult-Gerontology Primary Care Nurse Practitioner — not by a physician, and not by software. Meet your provider.
This page explains how our service works. It is general information, not a diagnosis, and it is not a substitute for an evaluation of your own situation. Whether testing, treatment or a prescription is appropriate is decided during a visit.
If this is an emergency — chest pain, trouble breathing, stroke symptoms, severe bleeding, or thoughts of harming yourself — call 911 or go to the nearest emergency department. See our emergency care notice.
