The short answer
If the decision can be made from a history, your readings, your medication list and your laboratory results, a virtual visit is usually faster and cheaper. If it depends on examining you, it needs an in-office appointment. Emergencies are their own category and should not wait for either.
Where each concern belongs
| Concern | Usual starting point |
|---|---|
| Blood pressure, cholesterol, blood sugar | Virtual visit plus local laboratory testing |
| Refills and medication questions | Virtual refill visit |
| Fatigue, sleep, mood, alcohol | Virtual visit with targeted testing |
| Erectile difficulty | Virtual assessment first; examination if indicated |
| Rash or skin lesion | Virtual if clearly visible; in person if it needs close inspection |
| Testicular lump or pain | In person — sudden severe pain is an emergency |
| Prostate symptoms | Virtual discussion, then examination or referral |
| Hernia, abdominal pain, joint injury | In person; examination is the deciding factor |
| Colorectal screening | Ordered virtually, performed at a facility |
What a virtual visit does well
- Establishing a baseline: history, risk factors and the right laboratory tests.
- Ordering testing locally with the cash price quoted before anything is ordered.
- Reviewing results with a plan attached rather than leaving them on a portal.
- Managing blood pressure, cholesterol and blood sugar over time with home readings.
- Telling you honestly whether you need to be seen, and where.
What in-person is for
Examination-dependent problems. For Presence members in Dallas–Fort Worth, scheduled in-office visits for ages 13 and over are at our North Richland Hills practice — $129 on Virtual Family Care, or two included each membership year then $99 each on Virtual + Local In-Office Care. We are not a walk-in clinic and we do not claim same-hour availability.
When it is neither
Some things need an emergency department now — the red-flag list above — and some need a specialist rather than primary care. Saying so early is part of a good visit. See when to get emergency care and choosing between urgent, primary and virtual care.
