The short answer
Emergency department for red flags. Primary care for everything ongoing, everything medication-related, and most new problems that are not urgent. Urgent care when something genuinely needs today's attention and your own practice cannot provide it — accepting that they will not know your history or your medication list.
What each setting is for
| Setting | Best at | Weakness |
|---|---|---|
| Primary care (virtual or in office) | Continuity, medications, results, chronic conditions, new non-urgent problems | Not for emergencies; not always same-hour |
| Urgent care | Today's minor injuries and illnesses, in-house testing, imaging on site | No relationship, no medication list, no follow-through |
| Emergency department | Time-critical illness: stroke, cardiac events, serious infection, major injury | Long waits, high cost, and disorienting for a frail older adult |
Why the threshold is lower in later life
Older adults often present differently. A serious infection may show up as confusion rather than a high fever. A heart attack may present as breathlessness or fatigue rather than classic chest pain. A minor fall can cause a significant bleed in someone taking a blood thinner. So symptoms that would be watched in a younger adult deserve earlier assessment here.
Start with primary care when
- The problem has been building over days or weeks rather than hours.
- It concerns a medication, a dose, a side effect or a refill.
- You have a result you do not understand — see what happens after an abnormal lab result.
- A chronic condition is drifting: blood pressure, blood sugar, weight, swelling.
- You are not sure. A virtual visit is a cheap, fast way to find out where to go.
How Presence fits
Presence is a primary-care practice, not an emergency service and not an urgent-care centre. Virtual visits cover the metro and every state where the provider is licensed; scheduled in-office visits for ages 13 and over are at North Richland Hills. We are not a walk-in clinic and we do not claim same-hour availability. If a problem needs an emergency department, we will tell you plainly — see when to get emergency care.
This page helps with choosing a setting. It cannot tell you how serious your own symptom is. When in doubt about anything on the red-flag list, choose emergency care.
