The same provider every visit: why continuity changes your care
Repeating your history to a new stranger is not just tiring — it costs you accuracy. This page explains what continuity of care actually does for you, how Presence structures it across virtual and local visits, and what continuity cannot promise.
Same local provider each in-person visit · One record across virtual and local care
The short answer
- What continuity means
- Continuity means one provider carries your history forward instead of meeting you cold. At Presence, local in-person primary care is delivered by the same provider each visit, and virtual and local care share one record.
- Why it matters
- A provider who already knows your baseline notices change. Subtle shifts — weight, blood pressure trend, how you describe fatigue — are only meaningful against what came before.
- Where it has limits
- Around-the-clock virtual care is delivered by a participating clinician network, so the individual provider on a 2am video visit may vary. Your record still travels with you.
- What you should ask any practice
- Will I see the same provider, how large is the panel, and who has access to my history when my provider is away.
- Next step
- Join, or read about the provider you would see in person.

How much continuity do you need?
- Membership primary care
You want one named provider who knows your history
Presence membership primary care, in the states where the provider is licensed.
- See Whole Family Virtual Care
You mostly need access at any hour, for anyone at home
Whole Family Virtual Care gives 24/7/365 household access nationwide, though it cannot promise the same clinician each visit.
- Get care now
You just need one problem sorted today
A one-time virtual visit from $19.99.
- Older-adult primary care
It is for an older parent with several conditions
Continuity matters most here — adult-gerontology trained membership care.
How this differs from a network telehealth visit
Both pathways are legitimate, and they are not the same product. Choosing well depends on whether you want a relationship or availability.
Presence membership care
Your visits are with Melissa, a board-certified Adult-Gerontology Primary Care Nurse Practitioner, in the states where she is licensed. She knows your history because she took it.
Whole Family network care
$19.99/month for everyone in the household, available 24/7/365 in all 50 states. Whichever network clinician is on when you need care sees you — we will not promise the same clinician each visit.
Using both
Plenty of households do: a relationship for ongoing care, and the network for a 2am fever or a visit while travelling.
If continuity with one clinician is what you are buying, choose membership care and say so — we would rather you pick the right pathway than be disappointed by the wrong one.
What continuity actually does
Medicine is largely the study of change over time. A blood pressure of 138 means one thing in someone who has always run high and something else entirely in someone who was 118 last spring. A provider seeing you for the first time has the number; a provider who has followed you has the trend, and the trend is where decisions live.
Continuity also reduces repetition errors. Every retelling of a history loses detail — the medication you stopped, the reaction you had, the test you already had done. When one provider holds the thread, fewer things need re-establishing and fewer things get missed.
Fewer repeated tests
A provider who knows what has already been ordered is less likely to order it again.
Better medication decisions
Your history of what worked, what did not, and what you could not tolerate stays in one place.
Faster judgement calls
"This is not like you" is only possible from someone who knows what is like you.
How Presence structures continuity
One clinical onboarding
A one-time $49 medical history review before your first in-person visit builds the baseline properly, rather than assembling it in fragments across appointments.
The same local provider
In-person primary care in the Fort Worth area is provided by the same board-certified Adult-Gerontology Primary Care Nurse Practitioner, for ages 13 and up.
One record across both channels
What happens in a virtual visit is documented in the same record your local provider works from, so a 2am fever is context at your next appointment rather than a lost episode.
Planned follow-up
Follow-up is scheduled as part of the plan instead of left to whether you happen to call back.
Being straight about the exceptions
Continuity you can count on
- The same provider for local in-person primary care
- One record covering virtual and local care
- Your history reviewed properly once, rather than reconstructed repeatedly
Where it varies
- Overnight and around-the-clock virtual visits, delivered by a participating clinician network
- Times when your provider is away and coverage is needed
- Care delivered by a hospital, emergency department or specialist outside Presence
Membership is not health insurance. Prices shown are membership prices, billed monthly on a 12-month commitment.
Questions people ask
Is continuity guaranteed?
For local in-person primary care the intent is explicitly that you see the same provider each visit. No practice can guarantee a specific individual is available on a specific day, and we will not pretend otherwise.
Does the 24/7 virtual provider see my history?
Virtual visits are documented in the same record your local provider works from. The individual clinician on an overnight visit may not be the same person each time.
Can my whole family have the same provider?
Local in-person primary care is for ages 13 and up, so an adult household can share a provider. Younger children are handled through virtual family care pathways.
What if I do not click with my provider?
Tell us. A relationship that is not working is a clinical problem, not an etiquette problem, and we would rather help you find the right fit than keep you in the wrong one.
Will my records transfer from my old practice?
You can request records from a previous practice, and we will review what you provide as part of onboarding. Timelines depend on the practice releasing them.
About this information
Written by the Presence care team for patients considering care with us. Presence care is delivered by licensed providers, including a board-certified Adult-Gerontology Primary Care Nurse Practitioner — not by a physician, and not by software. Meet your provider.
This page explains how our service works. It is general information, not a diagnosis, and it is not a substitute for an evaluation of your own situation. Whether testing, treatment or a prescription is appropriate is decided during a visit.
If this is an emergency — chest pain, trouble breathing, stroke symptoms, severe bleeding, or thoughts of harming yourself — call 911 or go to the nearest emergency department. See our emergency care notice.
