For the Self-Employed · 1099 · Owner-Operators
Healthcare for people who work for themselves
24/7 family healthcare, lower-cost labs, imaging and medications, with simple monthly options that do not require a traditional employer health plan.
From $19.99/mo · No HR department needed · Membership, not insurance
The short answer
If you are self-employed or 1099, household Presence membership from $19.99 a month gives you unlimited 24/7 virtual care, member pricing on labs and imaging, and plan-specific scheduled local care in North Richland Hills. It is not insurance and does not cover hospital, surgical, specialist or emergency care. HSA and FSA use depends on your plan.
- Who it is for
- Self-employed people, 1099 contractors and small owner-operators with no group plan.
- When it may not fit
- Anyone relying on this instead of major medical coverage. It does not cover hospital, surgical, specialist or emergency care.
- What it costs
- From $19.99 a month, billed monthly on a 12-month commitment; local visit economics vary by plan and are published before enrollment.
- What to do next
- Join, or price a year against last year's receipts.
In an emergency call 911 or go to the nearest emergency department.

The problem nobody solves for you when you work for yourself
When you are 1099, an owner-operator or a two-person family business, nobody negotiates your healthcare for you. There is no HR portal, no open enrollment reminder and no employer paying half the premium. What you usually have instead is a marketplace plan with a deductible in the thousands, or nothing at all — plus a strong incentive to skip care, because a lost workday costs you twice: the visit and the income.
That is the specific gap Presence fills. Not coverage for a hospitalization — a membership that makes the everyday and ongoing care you actually use cheap, fast and predictable, so a sinus infection or a blood-pressure recheck never becomes a scheduling and money decision.
Care that fits a working day
Unlimited 24/7/365 virtual visits by phone or video. Call from the truck, the job site or the kitchen table at 9pm instead of losing a billable morning to a waiting room.
Costs you can predict
One monthly membership price, no copays for included care and no claims. Labs, imaging and medications are quoted at member rates before you commit.
A provider who knows you
The same local provider each time, so your history and medications carry forward instead of restarting at whichever clinic had an opening.
Why this looks different from a marketplace plan
A high-deductible individual plan is designed around catastrophe. Until you hit the deductible, routine care is effectively self-pay — which is why so many self-employed households pay a premium every month and still avoid the doctor. Presence works the opposite way round: it starts with the care you use several times a year and makes that nearly frictionless.
Presence tends to fit you if
- You are 1099, a sole proprietor, freelancer, consultant or owner-operator
- You have a high-deductible plan and avoid using it for routine care
- You have no health plan today and want something real, not nothing
- Your income depends on your own hours, so lost days are expensive
- You want your spouse and kids on the same membership
- You already pay cash for labs, refills or urgent care visits
What Presence is not
- Health insurance, and not a substitute for a major-medical plan
- Coverage for hospital, surgical, emergency or specialty costs
- An ACA-compliant plan or minimum essential coverage
- A promise of specific savings on any individual lab, scan or prescription
- In-person care everywhere — local Presence visits are Fort Worth-area based
Many self-employed members keep a high-deductible or catastrophic plan for the big, rare events and use Presence for everything routine. We are happy to say plainly when a membership is not the right fit.
Self-employed membership options
Start with how many people need care. One person can start at $19.99/month; households usually start with the couple/family virtual plan. If Presence is going to be your primary care home with regular in-person visits, the unlimited plans stop counting visits altogether.

Where the money actually shows up
Everyday illness stops costing a day
Sinus infections, rashes, UTIs, pink eye, minor injuries and medication questions are handled virtually, included in membership, at whatever hour you are free.
Labs get cheap enough to actually order
Member cash-pay lab pricing through participating partners is often a fraction of billed pricing — see labs & imaging savings for examples and how quotes work.
Imaging gets quoted before you agree to it
X-ray, ultrasound, CT and MRI through participating imaging partners are priced up front where possible, which matters most when you are paying out of pocket.
Maintenance medications get boring
Long-term prescriptions are handled at partner pharmacy pricing where available, without an appointment just to get a refill.
Labs, imaging and medications are paid separately at the applicable member rate and are not included in membership dues. Savings vary by test, facility, pharmacy and location and are not guaranteed for any individual service.
What actually happens when you get sick and nobody covers your day
This is the part a marketplace plan does not solve. Three situations decide whether a self-employed household has healthcare or just insurance.
You wake up sick and you have work booked
Call before the day starts and get evaluated by a licensed provider by phone or video. If a prescription is appropriate, it goes to your pharmacy — you keep the job on the calendar instead of trading it for a waiting room.
Your kid spikes a fever at 11pm
On a household membership, you reach a provider right then rather than deciding alone whether it is an ER night. There is no copay for included virtual care, so the decision is clinical, not financial.
Something needs following, not just fixing
Blood pressure, cholesterol, thyroid, refills and recheck labs get managed by the same provider over time — with member cash-pay lab pricing so ordering a panel is not a $400 gamble against your deductible.
Presence does not provide emergency care. For a medical emergency call 911 or go to the nearest emergency department.
Where you work changes what you get
Virtual care travels with you anywhere Presence is licensed. In-person Presence primary care does not — and we would rather say so before you join.
- North Texas: you can use both halves. DFW direct primary care plus 24/7 virtual care is the full version of the membership.
- Illinois: virtual care year-round, with quarterly in-person care days — see Presence Illinois.
- Anywhere else: expect a virtual membership. Family virtual health explains exactly what is national and what is local.
If you employ anyone, read this too
Plenty of self-employed members eventually put a crew on. If you have even a handful of W-2 or long-term 1099 workers, the same membership can be extended to them as a voluntary or employer-funded benefit at group rates — a real benefit you can offer without buying a group health plan.
See employer healthcare benefits for group pricing and how implementation works, or call 817-900-6368 and we will tell you which side makes more sense for your situation.
Self-employed questions
Is this health insurance for self-employed people?
No. Presence is a healthcare membership. It covers primary and virtual care and gives you member pricing on labs, imaging and medications. It does not cover hospital, surgical, emergency or specialty costs, and it is not minimum essential coverage.
Can I keep my marketplace or catastrophic plan?
Yes, and many members do exactly that — a high-deductible plan for the rare, expensive events plus Presence for everything routine. The two are not in conflict.
Can I use HSA or FSA funds?
It depends on your plan and situation. What is generally eligible, what usually is not, and what to ask your administrator is covered on HSA/FSA information. We do not give tax advice.
Can my spouse and kids be on it?
Yes. The couple/family virtual membership covers a household, and Family Unlimited covers a family of up to five with unlimited covered local primary care; each additional child after five is $29/month.
I work all over Texas. Does virtual care follow me?
Yes — virtual visits work anywhere Presence is licensed in Texas, which is what makes this practical for field, trade and route-based work. In-person Presence primary care is at our practice in North Richland Hills.
Can I write it off as a business expense?
That is a question for your accountant. We can provide payment records; we do not provide tax advice or make any representation about deductibility.
Working for yourself shouldn't mean going without care
